Since contract participation with carriers can change, we suggest you verify our participation with us or with your insurance company by dialing the toll free Member Service number listed on the back of your id card. Your carrier may also provide access to a list of participating providers on their website. Your insurance carrier determines what services are applied toward your annual deductible, some services such as wart treatment with chemicals or liquid nitrogen, biopsies, or other services defined as office surgery by your insurance may be applied toward your patient responsibility/deductible (this is not an inclusive list – please check with your carrier regarding your benefits).
