Family Dermatology staff will ask patients to bring the following information at the time of visit:
We take most major insurances. Since contract participation with carriers can change, we suggest you verify our participation with us or with your insurance company by dialing the toll free Member Service number listed on the back of your id card. Your carrier may also provide access to a list of participating providers on their website. Your insurance carrier determines what services are applied toward your annual deductible, some services such as wart treatment with chemicals or liquid nitrogen, biopsies, or other services defined as office surgery by your insurance may be applied toward your patient responsibility/deductible (this is not an inclusive list – please check with your carrier regarding your benefits).
Family Dermatology is not part of the State Health Plan Clear Pricing Project. Because we do not participate your specialty copay will be higher. Please refer to your insurance card, provider directory and/or call member services with any questions.
